Yes, a Venue Rep name and signature are both required to submit sales to Luminate. The Venue Rep signature must match the Venue Rep name.
A phone number for the Venue Rep is also required.
The Venue Rep is the person Luminate will contact to verify your submitted Venue sales. The Venue Rep must be a third-party representative for the Venue itself - Not the Artist, Record Label, or Artist Management.
Luminate recommends adding an email address for the Venue Rep in addition to the required phone number. An email address is not required to submit your sales, however. If you add an email address for the Venue Rep, they will receive a copy of the Settlement.
